資格要求：1.如果投標人為中華人民共和國境內(nèi)注冊公司，需提供有效的營業(yè)執(zhí)照（具備獨立法人資格）、一般納稅人證明/稅務登記證、組織機構(gòu)代碼證復印件或證照合一營業(yè)執(zhí)照，投標時提供原件掃描件（原件備查）。如果投標人為分公司的，應具有有效的證照合一的營業(yè)執(zhí)照和總公司合法授權(quán)書。總公司與分支機構(gòu)只可一家參與投標，同時參與投標視為投標無效。如果投標人為事業(yè)單位的，投標人應經(jīng)全國各級事業(yè)單位登記管理機關(guān)核準登記或者備案，具有合法有效的事業(yè)單位法人證書。如出現(xiàn)單位負責人為同一人或者存在控股、管理關(guān)系的情況，相關(guān)投標均無效，招標人隨時有權(quán)作出拒絕投標、取消投標資格、取消授標、不簽訂合同或在合同簽訂后終止合同等決定，相關(guān)投標人須無條件接受，且由此造成的任何損失由投標人承擔。如果投標人為境外注冊公司，需提供有效的公司登記注冊證明。2.投標人應為本次投標產(chǎn)品的制造商。本次招標不接受代理商、貿(mào)易商、貼牌商、代加工工廠參與投標。本次接受所投貨物的制造商全資的銷售公司或控股的銷售公司，以及接受所投貨物制造商的母公司參與投標，此類公司視同為制造商，投標時需提供相應的證明文件。同一制造商僅允許一家投標人參與投標，否則相關(guān)的所有投標將被否決。3.投標人所投產(chǎn)品的制造商須具備有效的GB/T19001（ISO9001）質(zhì)量體系認證證書或API Q1證書，并可在中國國家認證認可監(jiān)督管理委員會網(wǎng)站(http://www.cnca.gov.cn/)核實。如所投貨物的制造商提供的質(zhì)量管理體系認證證書是國外第三方認證機構(gòu)頒發(fā)的，則網(wǎng)站查詢不適用。Qualification Requirements:1.If the bidder is a registered company in the People's Republic of China, it is necessary to provide a valid business license (with independent legal personality), general taxpayer certificate/tax registration certificate, copy of organization code certificate or three-in-one business license, and provide a scanned copy of the original (original for reference) when bidding. Checking is not applicable to certificates issued by foreign third-party certificate authority.業(yè)績要求：（開標時需進行信息公開）1.2020年1月1日至投標截止日（以合同或訂單簽署時間為準），投標人所投產(chǎn)品的制造商應具有1個合同的不小于22寸的機械堵漏管卡產(chǎn)品的銷售業(yè)績。2.投標人須按規(guī)定格式提交業(yè)績表，并提交相關(guān)業(yè)績證明文件。業(yè)績證明文件包括但不限于：1）銷售合同掃描件和2）到貨驗收材料。投標人所提交的業(yè)績證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時間、合同簽署頁（應有雙方簽署或蓋章）、制造商名稱、貨物名稱、尺寸及到貨驗收材料（至少包含：貨物驗收單或結(jié)算清單或?qū)慕Y(jié)算發(fā)票（如有清單須提供，須體現(xiàn)發(fā)票號；發(fā)票需包含合同號或訂單號或貨物名稱或買方賣方信息，且與合同或訂單一致）或買方（或最終用戶）接收證明或調(diào)試驗收證明或提單（含箱單和發(fā)票）或報關(guān)單或買方（或最終用戶）簽署的SAT（現(xiàn)場接收試驗）報告。若業(yè)績?yōu)橛唵危杼峁膮f(xié)議掃描件、訂單掃描件及訂單相應的到貨驗收材料。貨物驗收單或結(jié)算清單至少應有合同甲方簽字或蓋章。3.未提交業(yè)績證明文件，或所提供的業(yè)績證明文件無法體現(xiàn)滿足上述業(yè)績要求的，均視為無效業(yè)績。1.
